Cats refuse dirty boxes — top cause of 'going outside the box.' Scoop poop and pee clumps every day. Empty entire box and wash with mild soap every 1-2 weeks.

Try a different litter or box style
Some cats are picky. Switch one variable at a time.
Add another box (multi-cat homes)
Territorial cats may avoid a box another cat has used.
